Are you supposed to get into the building in the city before you go to kill your sibs in the end?

I take it were in amkethran here, which i would hardly class as a city, and the only major building in that dump would be the monastery. In which case, yes you have to get in there. How should become obvious at the time, but saemon havarian can help.
