I cranked the difficulty out to the end of the bar and it said Difficulty: Insane. How do you get to Heart of Fury?

You have to run the external configuration program.

Heart of Fury (HoF) mode does the following two things:
All monsters get 80 + 3x their normal HP.
A 8hp monster would have 80 + 3 * 8 = 104hp in HoF mode
A 30hp monster would have 80 + 3 * 30 = 170hp in HoF mode

Experence does a similar thing.
It goes by 2000 + 4x.
So a 1exp chicken is worth 2004 in Hof mode
a 3000exp mummey is worth 14,000 in Hof mode

I've done a solo HOF game with a fighter/mage/thief and a two person game multiplayer with a fighter/mage/thief and a ranger/cleric; both of them starting from first level characters. Its frustrating at the beginning but its definitely doable.

Uhmm...I hate to sound like a complete noob, but what does running an external configuration program entail? Is it a simple matter of editing the icewind.ini file?

The configuration screen is the same place that you would have chosen to play on insane level. It can be selected from the start menu. At the bottom left of the difficulty slider bar is a check box saying 'Heart of Fury mode'. Check this and you should be there.

@ Warboss :

Go to the start menu, then to the location where the IWD shortcuts are.

(most likely programs;black isle;icewind dale)

There should be a shortcut to the configuration file, where you can tweak things to your hearts content ;] (including setting HoF mode on!)

I didn't see you explicitly say so; so Ill ask: Do you have HOW installed? I just bought it, today ($4.99!!! Ye-e-e-EESSS!!!), and the HOW manual says that HoF mode is a feature added by HOW.
