I'm running windows XP with a patched game. After killing the Colanth, crossing the lava, etc., I was on the way back out from the lake and found myself in the cave with the path to the dragon. I come out of the passageway from the lake and into a large cave with a brick walkway up to a bridge then into the room where the dragon shield opens the door. No other way out. I ended up jumping off the bridge and somehow swam under water to the backside of the boulder puzzle. In other words, I came out of a waterfall, found the chest with the last shard and had to use the ring to melt the boulders to get out. This was totally backwards to the way the walkthrough was layed out.
