Yes, Virginia, there IS a third Diamond Eyes! After many games of ignoring the chest on the beach in Beyjin (between the entry cave and the spacewreck) i got the notion to check it out this time, and in it was a Diamond Eyes! Something else was in there that i've never seen before: a Pk Crystal! this is a Miscellaneous item for Psionics with AC +4, Psionic Blast (L4), HP Regen +1, 50% vs. Mental. The one i found had 7 charges, i dunno what it recharges to as i don't have or want a Psionic...
Given that i've only even walked near that chest a couple dozen times, there's no telling how often those items appear in it.

According to Flamestryke's site, some monsters also drop Diamond Eyes. But congratualtions! It's probably not an everyday occurance.

A pk crystal? Is it an item you wear(equip), or like a mana stone?

It's a Miscellaneous Item. Goes in the Ring-Necklace-etc slot.
