how do you put together the Silverblade and the Equalizer, I have all the pieces but cannot put them together.

You get some in game clues as to how to do this, but the answer is: Cromwell the Dwarf in the Docks will 'forge' the items for you (at a cost). - Curdis

Go to the docks, near next building large one towards the right (dock side) of the Mar'vea Guild House, there will be a side door where the dwarf will forge the item provided.
