Does all it do,make you replay the game but offer different NPC's,quests and areas?

So its BG all over again,but with extras?

Pretty much. But the extras (IMO) are worth playing BG again. In addition to the NPCs, quests, and areas, there are a lot of cool items. Also, DSOTSC makes a few "tweaks" to the some of the existing BG areas and battles. It keeps the game interesting.

You do have to have Tales of the Sword Coast installed to use DSotSC.
