It's so darn powerful, I'd imagine it could only be dropped in HoF mode.
My only guess is that maybe somebody drops it in the Severed Hand. My guess would be Iyachtu Xvim, who dropped an incredible axe in normal mode for me (Greataxe of Cleaving +5, with +2d6 slashing damage). I was very, very happy with this one!!!
