the hammer is in the astral plane. do haerDalis's quest to get it, it's a really cool quest and you get lots of good stuff.
the girdle is on a demon knight in the kuo-tua dungeon in the underdark.
the gauntlets are in the planar sphere.
haven't found the scroll with the hammers true name yet.
the equalizer gem you find early.
the hilt is in the mind flayer dungeon.
the blade is also in the underdark somewhere, i forgot where i found it, maybe in Ust Natha.
