This little square I got without any mod, and even with Diablo a couple of times. Simple reset&rerun always helped.
I think it has something to do with the init of graphics mode. Is 3D acceleration turned on? Maybe try to turn it off if nothing helps... Or reinstall DirectX, the thing is a main cause of nearly all problems in Windows
